The solution seems pretty simple to me. Consolidate all off-balance-sheet vehicles, and adjust accounting rules to prohibit recognition of any derivatives/guarantees/contingent payments which are not fully-collateralized for their gross face value (or alternatively, require the issuer to post 100% reserves).
Fractional reserving is the problem folks!
